The odd page number position is most likely caused by a 'Jason tab' so Jim's references are not going to directly address the problem.

The Jason tab (named after a horror movie character that never dies) is a tab that gets inserted by Word into each line of a TOC every time the TOC is updated. It is usually unnoticed except on lines where the text is short enough that it comes into play.

Assuming your TOC styles don't include that tab themselves, you can remove all the jason tabs by selecting the TOC and pressing Ctrl-Q to clear any local paragraph formatting. Unfortunately, this needs to be done every time you update the TOC. You can also automate this with a macro if you prefer.
