Combine Headings in Table of Contents on one line

See TOC Tips and Tricks by Suzanne Barnhill, MVP.

In particular, look at TOC Entries that do not appear in the document.
You want to be using Table Entry (TC) Fields as the basis for your TOC rather than headings.
