If you don't want them to be active checkboxes you can convert them to a square symbol. Otherwise, look into the old checkit and uncheckit macros used with the original fax forms.
Checkbox Add-In Add Interactive Toogle or Sequenced Object to Document That uses symbols instead of active checkboxes.
