Default Forms in Word

Good Morning,

I am trying to create a form in Word but I am facing two issues. The first is that I want the fill-in-the-blanks to say what content should be added in the line. Right now I have a line drawn and in the next line superscript that says (Jurisdiction Name). I want it to say (Jurisdiction Name) above the line, but so that users can just fill in in.

Second, I want the form created so when the user fills in that blank, it will fill in every other blank of the same value.

For example, the form might say:

"The (Jurisdiction Name) is interested buying a baseball team. The (Jurisdiction Name) will offer up to (Monetary Amount). Representing the (Jurisdiction Name) will be (Representative's Name)."

Then when the user inputs "Jurisdiction Name" as "City of Flint", all "Jurisdiction Names" become "City of Flint" automatically. Might someone explain how to do this?

I am using Word 2007 and Microsoft XP Professional.
