WAIT! How foolish of me. In the same place as the footnote or endnote separator (whichever yours are), the dropdown also includes "Foot/end note CONTINUATION Separator", which you can delete in the same manner.

I've got another problem with this manuscript. Word won't let me insert the index after the bibliography where it is supposed to go. The command for insert index is greyed out. Any help for this? I include some screen shots to show what's going on. I could only insert the index at the end of the document, so that's where it is now.
