Default Template "form field" & pictures - need help

Hello,



I am creating a template to standardize our test reports at work. To do this, I've used the "forms" toolbar with the "text form field" button.

This works great to keep the form standardized (no extra headings, font changes, etc.). However, I'm having two issues:

1) The text forms do not allow the writer to insert a picture. Is there any way around this?

2) I'd like a specific set of text at the bottom of page number. Text forms cannot be used in the footer. So the location of the text is directly dependent on the number of lines used in the "reason", "objective", and "summary" heading. Meaning, this text will never be in the same page on two separate documents (thus not helping my desire to have a "standard").

Please see attached. Any thoughts or suggestions?
