If you're using Word 2007 or later, you could set the document up with Text and Picture Content Controls and Quick Parts that the user could use to insert the ContentControls as required. To see how to do this, go to:

With a bit of field coding or the use of the 'different first page' layout option, you can limit the footer's appearance to the first page.
