I'm not able to duplicate your problem using Win 11 and Word 2021. When I create a link in Word with a pdf1 file extension, I'm first asked if I want to open it, and if I say yes, then a OS dialog box opens up that shows a list of installed programs. I can choose Acrobat, and it opens. I did not do any file association beforehand.
