After clearing the 4.7 cm tab stop from TOC4 and formatting any entry with TOC4, the “rogue tab stop” no longer appears, and the entry respects the settings of the TOC4 style. However, the original issue still persists.

If you look where the tab stop is in relation to the indent you will see that it is the same distance as the 1.78cm tab stop in Heading 4.
So, clear the tab stops from your headings and do yourself a big favour and set your numbering up correctly using a List Style. |

There are no "tab stops" in the Heading styles, as shown in the screenshot below.

There are in the document that you uploaded. How do you expect people to help you if the documents you upload aren't the same as the one you are having difficulty with?

I apologize for the confusion. The .docx file I provided is the original template, which I used to build my current document. The screenshots are from the current document.
I’ve tried to include enough screenshots to clearly describe the issue I’m encountering. I’m not sure why this problem has been so difficult to resolve. |

Your screenshots tend to be overwhelming, to me.
A document with the problem is far more helpful than a multitude of large screenshots. Attach a Sample File Demonstrating the Problem - Here's How Please reduce the size of the screenshots by about 50%. How to attach a screenshot in this forum. Insert them in the body of your posts with text explaining what each one is about. |

Attached is a revision of your sample document.
I modified the Title in Preface style to not have a first-line indent and change some of the tab stops in your TOC. I do not know if that reaches your problem. |

Thank you very much for the prompt help. Great! Changing the tab stop for TOC4 from 4.7 to 3.5 makes it work like a charm.

To get the hanging indent properly, if that is what you want in your TOC entries, I would adjust the tab stop and the hanging indent so that they are at the same position.
At the moment, the tab stop is further to the right compared to the indent.

The adjustments were made by clicking on one of the misbehaving entries and moving the tab setting in the ruler, then checking for others that were problems until none were found.
