The [1], [2], [3] issue would be best addressed in a separate question to get the best answers. I believe this has been asked and answered in the past.



You cannot get a footnote to repeat on two pages as a footnote.


You could add a couple of cross-references to the footnote, one to the number and one to the text after your table.
