The checkboxes and grey boxes are simply floating graphics sitting on the page. They are not associated with the table cells. They are nearly all anchored to the Question paragraph (Select that paragraph and note which ones get selected).
The question is do you expect the users to complete the form electronically or do you want them to print it out and write on the piece of paper. This layout is purely for printed form completion. If you wanted to complete the form electronically, you should delete all those shapes and replace them with Content Controls.
