Word 2003 does not have or recognize Content Controls.
Here is a screenshot where I selected and moved the table. It shows most of the graphics did not move with the table.

The one for Secure Loans is actually shading in the table cell rather than a separate graphic. You could use just a Table to handle shading. You could use Form checkbox fields instead of the box symbol, but if you want them checked or unchecked on the computer more is required.
