The basic problem is that, when you created the hyperlinks, they referenced a hidden Word bookmark related to the headings concerned. When you moved those headings with Track Changes on, the hidden bookmarks didn't go with them. Accepting the changes resulted in the bookmarks' deletion. Is there a reason you didn't use cross-references?
