Default Form Filling, setting up a standard letter to pop up with prompts to fill in certain areas

I am setting up some standard letters for work an Im trying to get word to prompt field filling in like in the image below?


I imagine it will be a content control but I haunt figured it our yet.


Hoping someone can help, please and thankyou in advance.
