Default How do I complete the attached

Please forgive my ignorance but I am attempting to complete an application form - and it mainly consists of Text Boxes that have radio buttons in them.


I have spent considerably time trying to put my mark on just one of the boxes.

The attached is a sample. I would be very grateful if someone could give me the correct simple method to get a Black Mark in one of the boxes please. I do not want to have to print the document and then use a pen in the 21st Century.
