Well, then write to the people who made the form and tell them you would like to fill it in on your computer rather than by hand.

It is not designed for computer filling in!

You could convert each of the current choices with fields, but it would be a lot of work and should really be done by the people who want the information.

Or, if you have Adobe Acrobat (not reader) you could save as a pdf file and tell Acrobat to convert it to a form. The results would be mixed.
