check boxes
All; I'm using a form created by someone else and the check boxes can not be checked. When I googled it; it instructed me to restrict the form. When I did this; I could not fill out the form such as type name and dept. I removed the table that seemed to be surrounding the text and attempted to restrict again so the check boxes could be check. But this does not work. If I remove the restrict; I can type in the form but the check boxes could not be check. Please let me know how I can make the check boxes checkable upon click.

Wrong kind of checkboxes. Those are legacy form checkboxes.
Instead use Checkbox Content Controls. Otherwise, you can right-check the legacy checkboxes and change them to checked. Not something you want to train your users to do. Create a Form Using Word Content Controls |

That suggests your document may have a combination of content controls and formfields. These were not designed to work together and using them in the same document can cause inconsistent behaviour. Without actually seeing the problem document, though, it's impossible to be sure. Can you attach a document to a post with some representative data (delete anything sensitive)? You do this via the paperclip symbol on the 'Go Advanced' tab at the bottom of this screen.

Your document uses formfield checkboxes, but lacks any text formfields for data entry so as to allow users to input the Requester Name, Requesting Dept., etc. details.
You should either replace the checkboxes with content control checkboxes, as Charles suggested, or add the necessary text formfields. Even if you replace the checkboxes with content control checkboxes, I'd recommend adding text content controls for data entry. Which approach you take depends on who is to use the document. Formfields are supported in all Word versions. Content controls are only supported on PC versions of Word from 2007 on and on Mac versions of Word from 2013 on. Even on PCs, content control checkboxes only supported from Word from 2010 on. Content controls are also not supported in the doc format (which is what you're currently using).
