There's one more trick, however, that I'm not sure how to accomplish.

I need to have the page number in the footer as well as the chapter title. When I add the page number, however, it replaces the styleref. I'm using this guide here:

How can I add the chapter titles AND the page numbers in the footer?

Note that the page numbers require the following:
* They need to start on a specific page other than the first page.
* They need to be placed underneath the chapter titles in the footer.

If I can figure this out, I'll have everything I need, and you will have saved me months, maybe even years, of work.
