Unless you want to get adventurous with user forms, the simplest approach would be to use mapped content controls (as in the attached document).



The simplest means of mapping controls, so that they may be cross referenced as with the 'name' control, is to use the second item at http://www.gmayor.com/BookmarkandVariableEditor.htm
