Default Sorting Index According to Page Numbers

Hello
Please pay attention and read the question carefully:

I have a document in which there are several proper nouns. These nouns are diffused in whole pages of the document.
Now I want to create an Index in which these proper nouns be sorted according to their pages, not typically according to proper nouns themselves alphabetically. The Index I want to be created is some thing like this:
'Page 124: John Smith, Jack Brown, Mayfair.
Page 125: London, Hitchcock, bellagio Road.'
Attend to this point that number of each page must be updated if an especial proper noun moves to another page. For example if the writer cut the 'London' from page 125 and paste it in page 124 and update the Index, it must remove from page 125 to 124 automatically.
I hope to explain the question completely. If it is not obvious, aware me to explain more about.
How can I do it? Has Ms-Word some thing to do it? If your answer is negative, tell me how can I do it. It is very important for me.
