is it possible to have a Word Form/Template and have data automatically populate it from a different word doc that is used to collect data?

I have several forms that need to be completed with the same information. I want to collect all the needed data in one document and have it transfer to the forms. As I change information in the data doc, the information in the form will also change. Does that explain it will enough?
