please help, need formating a .doc with simple fillable text boxes and picture boxes

I am sure this is simple but just cant get it .
What I am trying to achieve is, 12 in total, equal sized, rectangle shaped boxes. each box to have a much smaller box equal length on the top and the bottom of said box. boxes cannot touch. I want the ability to enter and remove text from the upper and lower boxes of each box and the ability to easily place different photos in each box. i need this for work ASAP. please see my example. and help guide me if you can
