First, "template" is a term of art (or jargon) in Word. It means a special kind of document that is used to create new documents. An online form is a document that gathers information and puts it where you want. Ideally you will make a template for an online form.
Templates in Microsoft Word If you create a template, someone using Office Button > New can use it to create a new document that can be edited and saved without altering the original. The next link is about to have access to such a template using the QAT: File New Variations in the Versions of Word There are a number of ways you can get data in a form and replicate it. See Repeating Data (Populating Multiple Like Fields). Forms can be made with more or less flexibility for editing. In some you can only type information in limited fields. Here is an article with links about using classic online forms.
