First, "template" is a term of art (or jargon) in Word. It means a special kind of document that is used to create new documents. An online form is a document that gathers information and puts it where you want. Ideally you will make a template for an online form.

If you create a template, someone using Office Button > New can use it to create a new document that can be edited and saved without altering the original.

There are a number of ways you can get data in a form and replicate it. See

Forms can be made with more or less flexibility for editing. In some you can only type information in limited fields.
