Default Trying to make a form for my company, need a little help

So, I'm trying to make a Training Worksheet for my company, and I'm having some trouble getting the formatting to work. I'm guessing a macro may be the way to go, but I'm unsure... In any case, I'm trying to make it so that when I need to add another line item, the four lines will appear next to it. For instance, say I have these three training items:


Phone system __________ _______ ________ _________
Email __________ _______ ________ _________
Fax Machine __________ _______ ________ _________


I want to be able to add a fourth one underneath and keep the four lines alligned. When I try to do this with columns it tends to screw all the lines up as soon as I add another line item.


Phone system __________ _______ ________ _________
Email __________ _______ ________ _________
Fax Machine __________ _______ ________ _________
Security __________ _______ ________ _________



I hope I explained what I'm trying to do well enough, if not, please let me know

*apparently the formatting on the forums doesn't let me make the lines vertical under each other... so you'll have to use your imagination
