Default Word Opening with Input Boxes

Hi all!

I have a document that keeps opening with input boxes (screenshot attached), almost looks like an old mail-merge or something. I need to stop these input boxes from appearing as there are a lot of them coming up whenever I save etc., but I can't see how. I thought it might be a Macro but I don't see any macros in the document.

Anyone know how to get rid of such a thing?
