Default Drop down menu with different value to display name

I want to create a drop down menu where the value shown after clicking the display name is different. For example when clicking the drop down menu you would see diagnoses "acute respiratory failure" but when you select it it will fill show the value "518.5". I'm trying to create a small billing card.

Is there any way to do this without writing a macro? Or maybe a selection list when I click it it will then populate a field? I am a typical Word user, so I have no idea of the complexities of use. I'm just learning to use the Developer tab, so please understand I'm a bit of newbie.

Thanks for any help.

Not if you want to be able to select one thing and have something else display in its place. If you're happy to have both the selected item and some variable text dependent on it display, that's possible with formfields and field coding.
