Page number XII, TOC shows 12

And here's your document back - fixed. Someone had manually formatted the page # with a '\* roman' switch, which Word doesn't do if you use the header's page # format dialogue.

Thank you Paul. Don't know if that is the same problem the OP has. I was the one who did the formatting.
