word 2007 can't insert index after bibliography

I've got another problem with a manuscript. Word won't let me insert the index after the bibliography where it is supposed to go. The command for insert index is greyed out. Any help for this? I include some screen shots to show what's going on. I could only insert the index at the end of the document, so that's where it is now.
