Take a look at the Letterhead Textboxes and Styles tutorial for a feel on how to make a borderless text box. I don't use line numbered pleadings and don't have time right now to make something up for you.
I enclose an old template. And yes, the numbers do start over on each page. That is how pleadings I've seen work. You have page numbers and then you have line numbers on the page.
