Can you provide a sample completed document that has at least a few entries so we can see what is meant to happen when there are some observations.

Your data input form has a matrix with 8 rows and 4 columns but the summary sheet has 20 rows. This doesn't align with 8x4=32 so I have no idea of what you expect to see in the summary sheet. If the summary sheet had 8 rows or 32 rows it might be more obvious what you are expecting but it should become clearer if we can see what a completed sheet would contain.
