Exclamation Using Outlook forms to create surveys- problem

All,

I am using a form in Outlook 2007 to send a simple survey to internal customers. When the customer receives the form, it is in a simple text format. Am I correct in assuming that this is due to Outlook '07 not allowing these type of files to be displayed? How can I correct this?
