Default Mail Merge with 4 datasets

I hope someone can assist with a work document I have been intermittently working on and off for months.


The document is a reminder document to be sent out to medical centres for children who are overdue for vaccinations.

. There is the individual's information (e.g. surname, given name, DOB etc)
. The name of the medical centre (which is in the same table in the Word document as the individual's information)
. The vaccinations the individual is overdue for
. The vaccinations the individual has had previously

The data can either be in one spreadsheet or 4 sheets

Unfortunately I don't have the skillset to figure this out. I have tried to modify other information I have found on this forum with no success.

I have requested support from the IT department and no luck..
