Matthew, please update your profile to indicate Office/Word 2007 rather than 2003. It will make answers to your questions more useful.
When I perform a mailmerge on your data using Word 2007 I get four records. They show up when I do print preview and also when I print to pdf and also when I merge to a new document. |

I do not yet understand why my word 2007 have issue.

Unsure why, we fix it now by remove all characters after last mergefield «LNO»

Basically, we changed all letter templates from .doc to .docx format, and seems this issue is fixed permanently.
