Default How to idiot proof a certificate

How do I create (upon opening of template) a dialog box that asks the viewer five questions eg description, recipient, department etc and then takes these answers and populates a certificate in styles/formatting of only my choosing. I do not want them to to touch the document in anyway as they will kill the formatting. I have tried a basic user form but I think they will not complete it properly. If I use a macro that returns an error if they have not completed all five questions i will be on the right track. I have searched but am confused as to what macro I should be using or whether there is even a better/easier way than a macro.
