Hi Steve
Perhaps a more easier option would be to use Excel. I have attached a very basic Mock Up of your document to show what can be done. Page 1 contains the patient records and page 2 contains links to the To-Do box on the first page. Anything entered in the to-do box on the first page is automatically entered on the second page against the relevant patient.
