Hi Jamal,
The code to put a document's name on the title bar is quite different from the code to update fields. They have nothing to do with each other. You asked for a document with all the code in place. See attached. The document is in Word's doc format, but you can save it in docm format if you want. Note: I've modified the code that handles fields in tracked changes, but you may not even need that.
