Yes these are legacy form controls without content controls.
They are added to the document by going to
"Developer" > "Legacy forms" > "Text form field"

This has happened to about 5% of our users over the last year - so it certainly isn't affecting everyone. It only affects the user's profile - if they log in elsewhere, or another user uses their machine, it works without issue.
