shortcut bold not working in single document

Since it is only one particular document, the context should not be Normal.dotm.

Change the 'Speichern in:' dropdown to the document in question and see if the shortcut is applied there. If you try to add it in that document you should get a message that tells you what the keyboard shortcut is already applied to.
