I need a little help please

Hi Everyone,

I scanned in a form I have. I's based on multiple choice questions, each answer is given a numeric value and at the end of the questions the totals are added then divided by 100. as an example:

check off only 1 box that pertains best to you:
question XXXXXXXXXX

answers
(check off box or radio button) xxxxxxxxx
(check off box or radio button) xxxxxxxxx
(check off box or radio button) xxxxxxxxx



question 2 xxxxxxxx

answers
(check off box or radio button) xxxxxxxxx
(check off box or radio button) xxxxxxxxx
(check off box or radio button) xxxxxxxxx






Each check off box would have a different numeric value and then at the bottom

Total=( add all question value up and then divide by 100)
