Default Table of Contents not generating correctly


I have a document that has a table of contents that was generated correctly weeks ago when I started this document. All of the sudden my TOC is all messed up. Attached is an example of what has it looks like. The entire TOC is showing the formatting codes and even when I toggle the codes, the indention is wrong and the format codes come back when I select update all fields. I even deleted the TOC and when I regenerate, it is still wrong. I know Word uses the Heading1 and Heading2, etc to generate the TOC and the document headers look fine. Does anyone know what I might have done to mess this up?
