In that case, you should use formfields and, once the setup is done, apply 'filling in forms' protection to the document. They're inserted via Developer|Controls>Legacy Tools>Legacy Forms, and the protection is applied via Developer|Protect|Restrict Editing>Editing Restrinctions:Filling in forms.
