What the document attached to your post actually contains is images of ActiveX radio buttons and their captions.

I can't imagine why anyone would distribute a form like that - unless it was an afterthought when they found out ActiveX controls aren't supported on Macs. Are you sure your attachment is 'as-distributed' and that you haven't saved it? Since you're using a Mac, I suspect what's actually happened is that the ActiveX controls have been converted to images.

As for asking questions about religion on an application form, I'd have thought that was a breach of privacy in many jurisdictions - unless it's for employment in a religious context.
