You can type text from the keyboard as in any other document, and insert more content controls, which you will find on the Developer tab of the ribbon.
If you want to cross reference controls (as with the name control) you can insert mapped content controls with the add-in I referred to earlier. You can do it without, but it is much simpler to allow the add-in to do the work for you. Whatever you insert or select in one mapped control is immediately reflected in the cross referenced control(s).
