I am pretty sure this can be done although can't work out how, but guess it may need some kind of database somewhere ... ideally via Office 365 online ... I work in the healthcare sector and need to be able to create a word document to allow me to copy and paste the content into a third party app.
What I am trying to achieve is creation of a form that has a set of yes no and drop down options, once the appropriate selections are made I am looking to insert the answers into a pre-formated word document with sentences, paragraphs and headings. As an example... the following questions could be asked which would then form apart of the paragraph below. Q1 : Was the person wearing appropriate clothing - yes / no Q2 : Is the person compliant with medication - yes / no Q3 : Has the person used illicit substances in the past (drop down with yes no, if yes present option of - 7 / 14/ 28 days Having answered the questions below the response would form part of the sentences below. Mr A was wearing appropriate clothing for the time of day and weather OR Mr A was wearing inappropriate clothing for the time of day and weather. Mr A is currently compliant with medication OR Mr A is non compliant with medication. Mr A denies illicit substance use OR / AND Mr A has used illicit substances in the last (selected from drop down) 7 / 14 / 28 days. If anyone can help out that would be amazing as I just don't know where to turn with this. ![]() |
