I am speaking from ignorance here. One thing I observe is that you are using (1) content controls (2) checkbox formfields (not content controls) and (3) ActiveX radio buttons. I would advise only using content controls in Word 2010 for your purposes. Content Control Custom Checkboxes I do not know why some of your plain text controls have chevrons and others do not.
