creating a booking form

Hi All



i am looking at creating a booking form for my work that will simplify the data that we usually get in emails - this results in a bit of email tennis untill we get the correct information

i want to be able to send out a booking form with required fields ect that they must complete and return

can anyone help me , i have attached what i was looking for. i am new to this so i am open to other design and options
