Default Form creation with repeating page template?

Hi all,

I'm trying to find a solution to a document i want to create but i don't know if its possible to do what i want, or if word isn't the right office program for it.

I need to create a 'software revision log' which will consist of a first page with the project information, a second page with the hardware information then a revision page which would then be repeated for however many revisions of the software there are.

Is it possible to create a document that consists of three pages but then as new pages are added it would use the page 3 layout as a template to repeat on all following pages?...without having to copy and paste it from the previous page?

I'm stumped on this one and searching online hasn't given me the answer. Any help much appreciated!
