Check your list of customs dictionaries. I found two, the default one, and one that read, "ExcludeDistionaryEng409.dc" I have no idea what it is. I did check it (to exclude it) and my custom (default) dictionary for the project I am working on started working fine and making the changes I wanted. I don't know if this will work for you, but you might try it.
