Solved it

I was able to solve this by manually entering links from the TOC line items to the appropriate headings. Before I was relying on the links automatically generated by word when I created the TOC.

Then I opened the document on a Windows machine and saved as PDF and the links stuck (there are issues with links in Word Mac when converting to PDF).
